The Community Care Information Management (CCIM) initiative is focused on providing clinical assessment tools and standard approaches to support evidence based decision making about patient care in the community care sectors.
The CCIM initiative is composed of 3 distinct streams – Business Systems, Common Assessments, and the Integrated Assessment Record (IAR).

Business Systems

Provides standards, processes and tools for collecting financial, human resources and statistical data to support the mandatory reporting related the Ontario Healthcare Reporting Standards.  Business Systems includes Management Information System (MIS) and Human Resources Information System (HRIS).

Common Assessments

Provides standard tools to collect clinical assessment information used by health service providers to support care planning and admission decisions.  Common assessments include the following:

  • Resident Assessment Instrument (RAI) Minimum Data Set 2.0

  • interRAI Contact Assessment

  • interRAI Community Health Assessment

  • interRAI Preliminary Screener

  • RAI Home Care

  • Ontario Common Assessment of Need

Integrated Assessment Record (IAR)

Provides health service providers access to electronically view patient centric assessment information in a timely, secure manner to support collaborative care planning.

  • Community Mental Health and Addictions Organizations

  • Community Support Services

  • Long-Term Care Homes

  • Small and Complex Continuing Care Hospitals
