The strategic vision of Community Care Information Management (CCIM) is to provide seamlessly-integrated, community-based client care where all service providers can securely share and access consistent and accurate information electronically. CCIM consists of two streams and a number of projects to support the Community Care sector.

Assessment Projects

By equipping health care providers with tools that can facilitate the collection and use of client information, the Assessment Projects are working towards creating an assessment system that is improving the quality of care Ontario clients are receiving. Through the integrated assessment record, health service providers servicing the same client access previous assessment information to support collaborative care planning and service delivery.

Business Systems

Business Systems projects having implemented and are supporting the maintenance of Ontario Healthcare Reporting Standards (OHRS)/ Management Information Systems (MIS) in the Community Care sector. OHRS/MIS establishes the framework that defines the standards for collecting financial and statistical data. The MIS Solution, Microsoft Dynamics GP, helps organizations meet these requirements. The Human Resources Information Systems (HRIS) project in Community Support Services (CSS) and Community Mental Health and Addictions (CMH&A) assists organizations across Ontario to meet their Human Resources and payroll needs. The HRIS solution is designed to enable organizations to automate HR and payroll processes and provide easier access to organizational data for reporting purposes.
