Under the strategic and operational direction of the Ministry of Health and Long-Term Care (MOHLTC), Community Care Information Management (CCIM) supports the delivery of business and technology solutions to the community care health sectors.

Stakeholder Management

  • Provide data to support Ministry Branches: Home and Community Care, Strategic Policy, Health Sector Funding Model, Performance Improvement and Compliance and Health Data
  • Support Health Service Providers (HSPs) to ensure their continued adaptation
  • Conduct data quality and training sessions to foster use of best practices, processes and policies, in patient care and to improve quality in data submission and information management

Standards, Processes And Tools

  • Service Level Agreements and Operating Level Agreements for the provision of application support services
  • Support the Data Sharing Agreement for sharing personal Health Information through IAR
  • Support financial and human resources processes for HSPs
  • Establish intake processes and implement service models for the provision of service delivery
  • Align IT Service Management processes with GO-ITS and ITIL Standards

Information Technology Services

  • IT sustainment services, such as ongoing maintenance and support of existing IT infrustructure and applications
  • Managed hosting services for Management Information Systems (MIS) and Human Resources Information Systems (HRIS)
  • IT business solutions delivery, application development and maintenance
  • IT service management and technical support, privacy and security risk management
